If you’ve recently had an MRI for chronic back pain, you may have seen the term "Modic changes" in your radiology report. While "herniated discs" and "spinal stenosis" are household names, Modic changes are a more specific and often misunderstood finding that relates to the health of your vertebral endplates.

The vertebral endplates are the "transition zones" where your spinal discs meet the bony vertebrae. They are made of thin layers of cartilage and porous bone. Their job is to allow nutrients to diffuse from the bone marrow into the discs, which don’t have their own blood supply.
When these endplates become damaged due to wear and tear, inflammation, or infection, the bone marrow inside the vertebrae reacts. These reactions are what we call Modic changes.
In 1988, Dr. Michael Modic identified three distinct patterns of these changes visible on an MRI. Each type represents a different stage of spinal degeneration.

Can Modic changes be reversed?
Type 1 (inflammatory) changes can sometimes progress into Type 2 (fatty) changes, which are generally less painful. However, once the bone has become sclerotic (Type 3), it usually remains that way.
Does everyone with Modic changes have pain?
No. Some people have Modic changes (especially Type 2) without severe symptoms. We treat the patient, not just the MRI.
Are Modic changes hereditary?
There is evidence that some people are genetically predisposed to faster disc and endplate degeneration, but lifestyle factors like smoking and physical activity play a much larger role.
